Lewis Yocum, Doctor Who Performed Surgery on Strasburg: “I Wasn’t Asked”
Remember last month, when Washington Nationals GM Mike Rizzo said that the Nationals were just following doctor’s orders (and I broke down how those comments had changed over time), and Scott Boras bloviated about how there could be legal ramifications if his star client was forced to pitch anymore when it would clearly be against legal advice?
Yeah, me too. From four weeks ago:
“We’re looking at the long term health of the franchise and for Stephen Strasburg,” said Rizzo. “We’ve got a plan, we’ve got a blueprint of how to do this. This isn’t Mike Rizzo’s plan, he didn’t go to Medical school but Dr. Lew Yocum did and Dr. James Andrews did. We’re taking their recommendations and putting them into place.”
Now, at the time, I expressed my doubts about the validity of any need to shut him down at an arbitrary number, and also expressed this at the end of the Tommy John post.
I haven’t seen any literature that supports the tact from the Nationals here, but if anyone has a link to a study on arm injuries and whether it is reasonable to work a player back by throwing 90-110 pitches in April and then just going cold turkey and starting up five months later, drop them in the comments. (Please don’t actually say the Verducci effect). I like to read studies. If I had time (which I don’t because of the NFL ramping up) I would dig in and find cases of Tommy John surgeries and innings pitched the following year versus career longevity.
Of course, when the national media focused on the Strasburg story, the cool kids pointed out that the neanderthals, like myself I guess, were going against science and doctors. Here’s one example.
Well, now, my ability to do math leaves us with just one thing on the other side, as well as people who match what it comes out of. Really tough call.
That’s because the Los Angeles Times actually spoke with Dr. Lewis Yocum, who performed the Tommy John surgery, and asked him about all this shutdown stuff. “I wasn’t asked,” Yocum said, saying he hasn’t talked to GM Mike Rizzo in over a year.
Yocum said that, had he been asked, he would not have been able to provide conclusive information about whether Strasburg’s long-term health would be best served by shutting him down.
“There’s no statistic as far as studies,” Yocum said.
Yocum went on to say that the decision basically stemmed from how they handled Jordan Zimmermann the year before, and that Rizzo set his own standard on that. The pitch limit did not stem from any medical directive.
In other breaking news, Scott Boras is full of s***. I’m sure that’s what Jack Kogod was referring to.
Mike Rizzo gets praise because, even if no one knows if this is the right thing, gosh darn it, at least he acts with conviction (until he claims it was doctor’s orders all along, by the way).
Well, Dr. Yocum wants you to know that it wasn’t his call. Like I said in previous commentary on the topic, I have no problem with protecting a young pitcher’s arm. I personally don’t think you do that by having him pitch over 100 pitches in April, starting in full go, then cutting him off cold turkey, then starting up again next April. I’m relieved to now know I’m not a troglodyte who disregards modern science.

There are some, though nothing complete/conclusive.
My reaction is the same as before. Your problem is with the paradigm of the way pitchers are handled, not just with Rizzo.
I think workload (for runners, pitchers, etc) should be handled like a sine wave, gradual increase/decrease. I wonder if pitchers who pitch over the winter are less likely to become injured.

this situation has been blown so fucking far out of proportion. this really shouldn’t even be a topic of discussion until the nationals a)failed to make the playoff, post-strassy shutdown, b) lost in the playoffs, or c) won the world series.

Did Strasburg look like he was tiring or not? His effectiveness (for whatever reason) was decreasing. You can look at Felix Doubrant as an example of an uninjured arm which struggled late in the face of new career highs in workload. I’d have loved to see him shut down the middle of the season, and then work back into a normal workload again.

Last two starts, his velocity was 95-97. He was erratic with his command, though.

You don’t need 5 months of rest to recover normal wear/strain, but you do need more than 4 days. Which is where inning restrictions designed to limit accumulated micro-traumas are important.

Outside of golf though I can’t think of a sport that has a greater mental aspect than baseball. Especially for pitchers. I don’t know if he avoids the news or not, kind of difficult to in this day and age, but if he got as sick of his team talking about shutting him down due to fragility, it could certainly affect his performance on the mound.

Would you be cool with the shutdown now, if they’d been more careful with him in the spring, monitored him for signs of fatigue, and then shut him down the same time because he appeared to be tiring, which absolutely increases the risk of injury.

Prudence, sure. In no way am I saying be reckless, but aren’t there a few TJ subjects who weren’t cared for like this, some subjects who went on to successful careers post-surgery?
My only point is no one knows the future. You could shut him down for a year, and he could still tear something. No plan is without some risk. Every time he winds up to throw he is taking some risk, even in long toss.

This is the incidence of Downs’ Syndrome by maternal age. Pretend it’s risk of UCL re-injury following surgery, by accumulated fatigue and micro-trauma level, for which you’d be forced to use pitches thrown as a stand-in. When do you shut him down? How much risk are you okay with? 4% isn’t much, but it’s twice 2%.

You sir, must NOT be familiar with human hunting. I find it very challenging to release 3-4 grown men in the wild and kill them armed with nothing more than a bowie knife and a blanket (less messy).
Human hunting might not require the mental game like golf does, but I promise, when you cover yourself in feces and track down a 30-something year old man. Maiming him to the point where he’s simply pleading for his life, there is a heavy mental aspect. Those screams for mercy make you think about letting him go. Of course, that would be silly.
Your brain is all like, “You’ve come this far, finish the job chap.”

Yes, if there were specific reasons to be concerned, sure, shut him down. This was an arbitrary number established by the GM based his beliefs before he ever pitched this year, and done in a way that doesn’t seem to me any better in preserving his arm.
All that will not happen now is that he will not get injured in October. You can’t get in a car wreck if you don’t drive.
I don’t think it impacts at all whether he gets hurt next May, or in 2015.

How much risk is permissible if it was a pitcher not named Strasburg? How about a pitcher paid half or less than half what Strasburg is paid?
I do wonder how much of this goes away if it was Zimmermann or Lannan in this situation.
I can’t help but wonder as well, where was this baseball-wide angst when Strasburg was throwing last September for a team that finished 80-81?
I know answering questions with questions is not nice. In hindsight, my decision would have been a) don’t use him at all in ’11; b) ease him through extended spring training in early ’12; and c) start his ’12 big-league year in May or June.

There was some angst. And I thought it was ridiculous. He had the surgery, a full recovery/rehab, and was ready to go. I think pitching last Sept actually helped him a lot this year, in built-up strength/stamina.

This makes no sense to me at all, SROD. What’s wrong with using him in 2011, with a workload that matched his progress and fitness level. He had plenty of time to recover from it all winter.
If he didn’t pitch at all in 2011, then he should have had an even lower limit this year.

And this only works if he pitches in the winter. Otherwise you are taking a young pitcher who hasn’t thrown in however long, ramping him up to full speed after an even longer layoff.
The biggest issue with protecting all (young and old arms) is that the workload is very on-off. I want to see a study on Winter Ball and affect on injuries.

My concern was/is, you have no utility of him pitching now. Regardless of what plan you have, you can’t use him when you need him the most. Which plan would have allowed the team to use him in the postseason this year?
If Rizzo truly was on some sort of a gradual, sine-wave schedule that he developed, they why bring in Dr. Yocum? If you’re Rizzo, last year just say “it worked for Zimmerman, we’ll do the same with Strasburg, regardless of team success or lack thereof”.

Got a friend who’s been a lifelong Expos fan and now Nats fan. He agrees with the Nats in shutting Strassburg down for the season. They did the Same with Zimmerman and he came back strong. Plus, I believe every starter has an ERA below 3.6 or something like that.

My friend believes the pitching is strong enough to carry them thru their playoff run. Would have been nice to has Strassburg pitching of course, but he we watched the game last Friday at the local bar and you could tell he was freakin whacked. Was getting pretty pissed off about pitches, that’s when my friend said it’s good to shut him down. We’ll see.

Far too late, but here’s what I have to say anyway.
This is merely one side of an interpretation of a situation with very little background information. The other side is: The pitch limit did stem from medical directives, specifically Rizzo having spoken to Dr. Yocum about recovery for not one (Strasburg) but two (Zimmermann) pitchers. Both sides are equally valid.
I’m sure Yocum gave Rizzo suggestions on how best to see his pitchers recover. When it comes down to it, Yocum is the surgeon and Andrews is the guy the Nats go to for consultations.
So, again, without more information there’s no point to all of this because it can be argued with equal logic either way. And that brings us to the ridiculous game of Let’s Play Doctor!
