Former Argentina international Leo Ponzio suffered a ruptured hemorrhoid while playing for River Plate. He was fine, aside from the profuse bleeding. He reportedly had to change his shorts three times.

This happened to Marcos Senna with Villareal about 6 years ago in the Champions League. Or at least I assume that’s what happened because he had bloody shorts just like this guy.
